How North Shore Hearing collects, uses, and protects your personal and health information, including text messages and information covered by HIPAA.

North Shore Hearing, LLC operates an independent audiology practice at 7040 N. Port Washington Rd., Suite 401, Glendale, WI 53217. This policy explains what we collect, how we use it, and the choices you have. It covers this website and the care we provide in the office and through mobile visits.

Information we collect

Information you give us

When you contact the office, ask about an appointment, or become a patient, we may collect your name, address, email, phone number, date of birth, insurance details, and the health information needed to provide audiology care.

Information collected automatically

When you visit this website, we may record basic technical details such as your IP address, browser type, the pages you view, and the time of your visit. We use this to keep the site working well and to understand what visitors find useful.

SMS and text messaging

If you give us your mobile number and agree to receive texts, we may send appointment reminders, scheduling confirmations, and replies to questions you send us.

How you opt in

We only text patients who have opted in, either on a form, in writing, or by texting the office first. We do not share your mobile number with third parties or affiliates for their own marketing.

Frequency and rates

Message frequency varies with your care and your requests. Message and data rates may apply, depending on your mobile carrier and plan.

How to stop

You can stop texts at any time by replying STOP to any message from us. Reply HELP for help, or call the office at 414·906·1145. After you reply STOP, we may send one message to confirm that you have been unsubscribed.

We keep the content of text messages limited. We avoid sending detailed health information by text, and we follow HIPAA whenever protected health information is involved.

How we use your information

We use the information we collect to:

  • Provide and coordinate your audiology care and treatment.
  • Schedule appointments and send reminders and follow-up details.
  • Process payments and submit insurance claims.
  • Answer your questions and respond to your requests.
  • Meet legal and regulatory requirements, including HIPAA.
  • Keep this website working and improve the patient experience.

HIPAA and protected health information

As a healthcare provider, North Shore Hearing follows the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is handled under our Notice of Privacy Practices, which we give you at your first visit and provide on request.

We do not sell or rent your PHI. We share it only as HIPAA allows, for example with other providers involved in your care, with your health plan for billing, or when the law requires it. Any outside service that handles PHI on our behalf must agree in writing to protect it.

Cookies and analytics

This website may use cookies, which are small files stored on your device, to remember your preferences and keep the site running. You can set your browser to refuse cookies, though some parts of the site may not work as expected.

We may use a privacy-minded analytics service to see how the site is used. Where we use one, it is set up to avoid collecting information that identifies you.

Your choices and rights

You can ask to see, correct, or delete the personal information we hold about you, within the limits of applicable law and our HIPAA obligations. You can opt out of text messages by replying STOP, and you can unsubscribe from email using the link in the message or by calling the office.

To make any of these requests, contact us using the details below. We will respond within the time the law allows.

Changes to this policy

We may update this policy as our practice, our technology, or the law changes. When we do, we post the new version here with a revised date at the top. Please review it from time to time.
